Brains is a food item that gives you 40T and the Brainy status effect upon consumption (unless you have a Hobola infection). You only obtain this food item from the Dark Pit.

However, brains are gross to eat and require you to be infected with Hobola or have the zombified status effect to consume.

As of October 2015 you can no longer eat brains while already infected with Hobola during the Dark Pit event. Eating them with Dead Man Walking will give you a new Hobola infection (up to 5 times).
